The Social Worker HealthySteps Specialist will be integrated into the primary care team to promote healthy emotional, social, and physical development in young children from birth to age 5. The HealthySteps Specialist works alongside physicians, nurses, and clinic staff to provide comprehensive, family-centered care focused on early childhood development, behavior, and family well-being.
The Social Worker will provide onsite mental health services, including psychosocial assessment, documentation of interventions/referrals made.
Preferred experience working with at-risk families and experience in early childhood development, behavioral health, or family-centered services.
Assist with implementation of treatment plans with patients.
Work closely with medical team and staff to support referrals for services.
Manage a caseload of patients.
