About the role

The hospice Registered Nurse will perform patient assessments, develop care plans, and manage a caseload of up to four patients daily. They will also ensure compliance with hospice regulations and maintain effective communication with the interdisciplinary team.

Atlantic Visiting Nurse is seeking an experienced Hospice Registered Nurse to join our dedicated interdisciplinary team.

As a Hospice RN, you will build meaningful relationships with patients and families while providing expert clinical care in the comfort of their homes.

This autonomous role combines case management, patient advocacy, clinical assessment, education, and collaboration with physicians and the interdisciplinary team to ensure every patient receives compassionate, individualized care throughout their hospice journey.

Principal Responsibilities: Clinical Assessment & Patient Care Perform comprehensive nursing assessments to evaluate patients' physical, emotional, and psychosocial needs.

Determine patient appropriateness for hospice services.

Develop, implement, and continuously evaluate individualized plans of care.

Monitor patient progress and revise care plans based on changing clinical needs.

Provide symptom management and evidence-based nursing interventions Case Management: Serve as the primary Case Manager for an assigned patient caseload.

Coordinate care with physicians, social workers, hospice aides, chaplains, therapists, and other interdisciplinary team members.

Prioritize patient visits based on clinical needs.

Participate in interdisciplinary team (IDT) meetings and communicate patient progress.

Perform Home Health Aide supervisory visits in accordance with regulatory standards.

Patient & Family Education: Educate patients and caregivers regarding disease progression, medications, symptom management, and hospice philosophy.

Adapt teaching methods to meet the unique learning needs of patients and families.

Document education provided and evaluate understanding throughout the continuum of care.

Documentation & Regculatory Compliance: Complete timely and accurate clinical documentation within the electronic medical record (EMR).

Complete admission, visit, discharge, and case management documentation in accordance with agency standards.

Demonstrate knowledge of hospice regulations, Medicare guidelines, and insurance reimbursement requirements.

Medication & Safety Management: Perform medication reconciliation during patient visits.

Maintain current medication orders and monitor effectiveness.

Demonstrate sound pharmacology knowledge and safe medication administration practices.

Promote infection prevention and maintain a safe environment for patients, caregivers, and staff.

Quality Improvement & Professional Development: Participate in Performance Improvement initiatives and utilization review activities.

Assist with orientation and mentoring of new employees.

Cross-train to support departmental needs and share clinical knowledge.

Attend staff meetings and continuing education opportunities.

Hospice

At Atlantic Visiting Nurse, we believe that those living with advanced illnesses should spend their final days in comfort and peace.

Our compassionate, specialized care is designed to minimize patient suffering and to support families and loved ones in their roles as caregivers.

As one of the first hospices in New Jersey and the United States, we have more than 40 years’ experience serving private homes, nursing facilities and assisted living facilities in Essex, Morris, Somerset, and Union counties.

When patients and their primary care physicians agree that curative treatment is no longer appropriate or desired, a referral to Atlantic Hospice may be appropriate.

Our hospice team works with referring doctors to develop individualized care plans that manage pain, control symptoms, and ensure dignity and respect in the final days of life.

We also have nurse liaisons at Morristown Medical Center, Overlook Medical Center, Newton Medical Center, Chilton Medical Center, and Hackettstown Medical Center, as well as other health care facilities in the area, to answer questions and ease the transition to end-of-life care.

Requirements

Candidates must have a current NJ RN license and a BSN, with a preference for at least one year of experience in a relevant care setting. Experience with EMR systems and strong critical thinking and communication skills are also preferred.
