About the role

The Occupational Therapist will provide skilled home-based therapy care as directed by the attending physician, focusing on developing individualized plans of care and improving patient function through activities and exercises. Responsibilities also include educating patients and families, minimizing re-hospitalization risks, and supervising Certified Occupational Therapy Assistants (COTAs).

  • Monday through
  • Friday 8:30am to 5:00 pm/ No weekends or holidays required.

Territory: Logan County, Champaign County and Clark County

How you'll make a difference

Care and compassion are at the heart of what you'll do as an Occupational Therapist (OT). You'll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population. You'll provide home-based therapy care for patients as directed by the attending physician. Interacting with patients' families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey. Home care provides context and real-world perspective about what will really help patients restore their health.

How you'll work

You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.

Major areas of responsibility

  • Plan of
  • Care: Provide skilled occupational therapy as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Develop an individualized OT plan of care and make adjustments as needs change.

Evaluation

Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.

Patient Outcomes

Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.

Minimize Patient Risk

Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.

Family Educator/Advocate

Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy. Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.

Supervision

Oversee and supervise total care of patient provided by COTA.

Communication/Collaboration

Communicate with the COTA on supervisory visit results.

Policies

Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.

Rules and Regulations

Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.

Operations

Attend in-service training and mandatory agency meetings.

Requirements

Candidates must possess a current Occupational Therapist license in the state of employment and a minimum of one year of experience as an OT, preferably in an acute care setting, with home care experience being a plus. A valid driver's license, auto insurance, and the capability to meet all physical demands of the role are also mandatory.
