Provide high-quality nursing care to patients in a long term care and short term rehab setting.
Administer medications, perform wound care, monitor patient vital signs, and assist with treatments as directed by healthcare providers.
Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents.
Educate residents and their families on health management strategies and treatment plans.
Ensure accurate documentation of patient information and treatment procedures in electronic health records. **Requirements:*
Valid LPN license in PA.
Ability to work effectively in memory care units and long-term care facilities.
