The Physical Therapist evaluates patients' functional levels and develops therapy plans while providing direct care in home settings. They also supervise PTAs and Home Health Aides, ensuring proper documentation and communication with the care team.
Candidates must have a Bachelor of Science in Physical Therapy, with a preference for a master's or doctorate. A minimum of two years of clinical experience is required, preferably in home care, along with a valid Connecticut license and CPR certification.

The Physical Therapist is a qualified professional who directs, supervises, evaluates, and provides direct care to patients and provides physical therapy services to patients in the home according to State and Federal regulations under the direction of the attending physician.
