The Nurse Practitioner or Physician Assistant will provide primary care services, including obtaining medical histories, performing physical examinations, and developing therapeutic plans for patients. Responsibilities also include ordering diagnostic procedures and providing health and illness prevention recommendations.
Candidates must have 3-5 years of primary care practice experience and be a graduate of an accredited Nurse Practitioner or Physician Assistant program. A valid license in Vermont and strong patient care skills are also required.
Community Health is a multi-site FQHC serving 80% of the primary care needs in our community (central Vermont). As the largest FQHC in northern New England, we have a large footprint that provides us with the opportunity to influence the evolving, local, state and regional health care environment.
Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.
Community Health Castleton uniquely provides the community with access to Primary Care, Behavioral Health, and Care Management services for the whole family. Primary Care services include on-site Behavioral Health providers, Social Workers, and Care Management, along with preventive programs like diabetes management and education. Castleton Express Care, located within the same facility as Community Health Castleton, is open seven days a week, for the treatment of minor illnesses and injuries for adults and children.
The town of Castleton is home to Lake Bomoseen and Castleton University. Picturesque, historic downtown Rutland, the third largest city in Vermont, is the county seat and about 20 minutes from Castleton. The Green Mountain National Forest, Killington, and Pico ski resorts are about a 45 minute drive from the quaint, small town of Castleton, and a short drive to New York’s Saratoga Springs and Lake George area.
