The Case Manager is responsible for managing and coordinating comprehensive Home and Community Based Services, ensuring they meet consumer needs according to CareSource contractual expectations. This includes conducting comprehensive assessments, developing person-centered care plans, and arranging service delivery through collaboration with various providers.
Applicants must be a Registered Nurse (RN) or Licensed Social Worker (LSW) with a current, unencumbered license and at least one year of prior experience in home health care, medical social work, or geriatrics. Candidates must also possess a valid driver's license, proof of insurance, and the ability to pass a criminal background check and drug screen.
Under the general supervision of the Director of Home and Community Based Services and/or the CareSource Supervisor, the Case Manager (CM) is responsible to know and abide by CareSource program contractual expectations. The CareSource CM is expected to manage and coordinate comprehensive services and assure that they are appropriate and meet the needs of the consumer to support them.
Conducts face-to-face comprehensive assessment, as per specific program requirements, needs including preventative health testing (physical, cognitive, social, emotional, financial, spiritual, cultural, and environmental) with the consumer/family system, obtaining and evaluating the care plan and services to determine program eligibility, needs, and options. Develops a comprehensive person-centered care plan addressing the consumer’s needs; including the goals and outcomes. Arranges for service delivery through a process which includes collaboration with service providers (including informal caregivers, volunteers and outside resources) Conducts initial assessment, in-person visits according to acuity and a reassessment per program guidelines. re-evaluates consumer goals, objectives, acuity, service plans and program eligibility per program requirements. Evaluates the health and welfare needs of the waiver enrolled individual to assist in ensuring the individual’s welfare. The case manager is responsible for assisting the waiver individual in identifying risks to health and welfare associated with their decisions and explores options available to them to meet the identified need(s) and development of a Health & Safety Action Plan (HSAP) if necessary. Must have the following abilities to perform the responsibilities: Ability to effectively communicate with the general public, in person, by phone, and via email, Ability to comprehend written material related to essential job functions, Ability to accurately document facts related to essential job functions, Ability to comprehend, recall, and apply facts related to essential job functions, Ability to analyze, evaluate, and implement a reasonable course of action based on available information, Ability to lift/carry 20 lbs. upstairs, Ability to travel to community and individual locations within our four-county service area (Ashtabula, Mahoning, Trumbull and Columbiana counties), Ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio and Ability to ambulate enough to access private homes/locations which may not be readily accessible. Qualifications Job Requirements: Applicant must be a Registered Nurse or Licensed Social Worker with current, unencumbered license, with at least one year’s prior experience in home health care, medical social work and/or geriatrics. Demonstrates working knowledge and skills necessary to provide the assessment and care management required by the consumer. Applicant must demonstrate flexibility in schedule and in adapting to rule changes and productivity changes and possess an understanding of 1.) Medicare, Medicaid and third-party reimbursement and 2.) Community service delivery systems and the ability to link consumers to community resources. Applicant must possess a valid driver’s license and proof of automobile insurance. Must also pass a criminal background check and drug screen.
