• Therapists (OT) responsibilities include: Conduct assessments to evaluate patients’ physical conditions and functional abilities Develop individualized treatment plans based on assessment findings and patient goals Provide hands-on therapy including therapeutic exercises, functional activities, and self‑care skill training Educate patients and families on proper techniques, safety, and rehabilitation strategies Collaborate with interdisciplinary healthcare professionals to coordinate patient care Maintain accurate and detailed documentation of patient progress, treatments, and plan updates Adjust treatment plans based on patient response and progress toward goals Attend staff meetings and in‑service programs as needed Requirements for the Home Healthcare Occupational Therapist Valid Occupational Therapy license to practice as an OT Knowledge of medical terminology, physiology, and anatomy Experience in home health or a clinical setting (preferred but not required) Strong communication skills for working effectively with patients, families, and interdisciplinary healthcare teams Ability to work independently and as part of a team Compassionate, patient‑centered mindset with a genuine desire to help others
