The Case Manager assists clients in identifying treatment and recovery goals by providing supportive services and completing necessary documentation like assessments, crisis plans, and individualized service plans. They collaborate with the clinical team to coordinate care across various providers and conduct outreach/follow-up for services, ensuring all prescribed plan elements are implemented.
Candidates need a minimum of a High School Diploma/GED, with experience requirements varying based on education level, ranging from 1.5 years of behavioral health experience to a Master's degree in the field. Essential requirements include a valid Arizona driver’s license, meeting minimum driving experience, possessing or applying for an Arizona Fingerprint Clearance card, and basic computer proficiency.
