This role involves providing outpatient and aftercare services to patients, including conducting individual or group counseling sessions and performing necessary chemical dependency assessments to determine the appropriate level of care. Essential duties also include developing treatment plans with patients, facilitating placements in treatment facilities when required, and managing necessary case follow-up and referrals.
Candidates must possess a minimum of a high school diploma or GED and hold a current CAADC or similar certification, with an Associate's degree being preferred. A minimum of three years of related work experience is preferred, along with the ability to pass required background checks, physical exams, and drug screening.
Reports To: Substance Use Disorder Counselor Manager FLSA Status: Non-Exempt Classification: Regular, Full-time Schedule: Monday - Friday, 8am - 4:30pm Location: Onsite, position may be assigned to work at both IHC locations Grant End date: 09/29/2027 SUMMARY: Provides outpatient and aftercare services to patients. ESSENTIAL DUTIES AND RESPONSIBILITIES: The information in this job description indicates the general nature and level of work performed. Duties and responsibilities may change (be assigned or removed) at any time with or without notice. 1. Provides individual or group counseling. 2. Performs necessary chemical dependency assessments; able to use ASI and or ASAM assessments to identify level of care. 3. Treatment plans will be developed and agreed upon by both patient and counselor. Treatment plans may also include case management services. 4. Provides individual chemical dependency counseling. 5. Facilitates placement in a youth and adult treatment facility when necessary. 6. Follows patient progress through necessary case management. 7. Facilitates needed referrals for patients to internal programs and outside agencies. 8. Maintains continuous review of referrals to ensure clients services in an efficient, expeditious and professional manner. 9. Where assigned to a grant funded project will execute objectives as they are established by the grant. 10. Provides community substance abuse prevention education or programs. 11. Counselors will collaborate with other IHC departments and outside agencies for the provision of treatment services. 12. Participates in department development activities, conferences, seminars, and workshops programs as directed. 13. Assists with the development of department communication guidelines with agencies and schools. 14. Maintains client case files in accordance with both HIPAA, IHC regulations and potential third-party payer. 15. Participate in outreach activities. 16. Position may be re-assigned during activation of EOP. SUPERVISORY RESPONSIBILITIES This position has no supervisory responsibilities. TRAVEL & DRIVING REQUIREMENTS · Travel may be required for this position. · Driving is required for this position must be comfortable driving around our service area. EDUCATION AND/OR EXPERIENCE · Minimum high school diploma or general education degree (GED) required. · Current CAADC or similar certification required. · Associates degree preferred. · 3 years related work experience preferred. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES · Knowledge of the principles and practices of counseling techniques. · Knowledge of community resources and referral agencies. · Knowledge of group dynamics. · Skill in counseling. · Skill in interviewing and communicating to elicit information and cooperation from patients. · Ability to write reports, correctly reflect treatment and document patient's progress. · Productivity Software: Microsoft Office 365 (Email, Calendar, Word, Excel, Power Point). · MS Teams (video conferencing and meetings), and Share Point. · Databases: NextGen or EPIC electronic health records. · Utilization of Paycom, Paycom Learning, and PolicyTech systems. CERTIFICATES, LICENSES, MEMBERSHIPS AND REGULATIONS · Current CAADC or similar certification required. · Completion of Grant funding training within first 3 months of hire date. · A valid California driver’s license and a good driving record will be required at the time of appointment and must be maintained throughout employment. · Current AED CPR certification or attainment within 6 months of hire. · Applicants must successfully pass a pre-employment physical exam, tuberculin skin test or x-ray and urine drug screen test to include marijuana. · Health must be adequate to perform all duties of the position. · Must pass criminal background check.
