This includes performing initial assessments, developing patient-specific plans of care, initiating therapy procedures, and communicating changes to the physician.
Candidates must possess a degree from an accredited Physical Therapy program, with a DPT preferred, and hold a current valid Massachusetts Physical Therapist license along with active American Health Association BLS certification. A minimum of two years of physical therapy experience is required, with at least one year specifically in public health or home care, and OASIS experience is preferred.

BILH at Home - North Shore Area Schedule - M-F 8am-4:30pm What You’ll Do: The physical therapist plans, organizes, collaborates and directs home care services. The professional therapist builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities in accordance with home care regulations and payer guidelines. More Specifically: Coordinate care for assigned patient caseload including but not limited to: delegating to and supervising of PTA staff (if applicable), processing patient condition change visits, and completing OASIS time points. Complete and document an initial assessment of patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illness(es) and develop a patient-specific plan of care to establish goals incorporating therapeutic, preventive, and rehabilitative actions by including the patient and the family in the planning process. Initiate the plan of care and makes necessary revisions as patient status and needs change. Initiate appropriate preventive and rehabilitative therapy procedures. Administers medications and treatments as prescribed by the physician. Communicate with the physician regarding patient needs and report any changes in patient condition; obtain/receive physician’s orders as required. Practice confidentiality principles set by the agency and federal HIPAA guidelines. Demonstrate proficiency in OASIS completion; complete documentation in accordance with agency timeliness policies. What You’ll Need: Degree from an accredited program in Physical Therapy; DPT preferred. Current valid Massachusetts Physical Therapist license; Active American Health Association BLS certification; 2+ years of physical therapy experience, at least one of which is in the area of public health or home care; OASIS experience is preferred. Pay Range: $80,080.00 USD - $107,777.00 USD The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
