This position involves assessing patient needs, reviewing care plans, and helping individuals access appropriate healthcare and social services by addressing social determinants of health. Key duties include conducting outreach activities like home visits, educating community members on programs, and maintaining accurate documentation of all interactions.
Candidates must be bilingual in Spanish and English, possess a high school diploma (Associate's preferred), and obtain CHW certification within six months of hire. Required experience includes 1-3 years in a clinical/healthcare setting preferred, along with the ability to work a flexible schedule including required evening hours during the growing season and occasional weekends.
Kinston Community Health Center is seeking Community Health Workers to join our Outreach Team. We are hiring two full-time positions for individuals passionate about connecting people with the healthcare and resources they need to thrive.
This role is community-facing and relationship-driven, working directly with individuals and families to improve access to care, address social determinants of health, and strengthen community health outcomes.
Evening availability is essential. Due to the seasonal nature of our outreach work, regular evening hours are required during the annual growing season, and candidates must be able to work nights during this time. Occasional evenings and weekends may also be required throughout the year as community needs arise.

This position supports and enhances Kinston Community Health Center’s Outreach Team by assessing patient needs and reviewing care plans to help individuals access the most appropriate level of care within the correct department or service area. The role also focuses on addressing broader social determinants of health, working to bridge gaps between vulnerable populations and essential health and social service systems. Through outreach, coordination, and advocacy, this position plays a vital role in improving health outcomes and promoting equitable access to care.
