The Licensed Practical Nurse will provide skilled nursing care in patients' homes as directed by the attending physician, focusing on compassionate quality care and positive patient outcomes. Responsibilities include monitoring patient conditions, administering prescribed medication, and educating patients and families on disease processes.
Candidates must possess a current Licensed Practical Nurse (LPN) license and have a minimum of one year of experience as an LPN in an acute care setting, with home care experience being preferred. A valid driver's license and auto insurance are also mandatory.

HOW YOU'LL WORK:You'll provide skilled nursing care in a patient's home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions and notify case manager of any changes in the patient’s condition. Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions. Family Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
