The Registered Nurse Case Manager will observe and monitor patient conditions, perform OASIS assessments, and create individualized care plans while administering prescribed medications and minimizing re-hospitalizations. This role also involves coordinating multidisciplinary care, educating patients and families, and ensuring compliance with documentation protocols and agency policies.
Candidates must possess a current Registered Nurse license and a minimum of one year of RN experience, preferably in an acute care setting, though home care experience is preferred. A valid driver's license and auto insurance are mandatory, along with the ability to meet all physical demands of the role.

This role offers autonomy, so time management and organization are essential. You will be responsible for duties within the full scope of practice as outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (training provided if needed) to create and adjust individualized care plans. Minimize Patient Risk: Administer medications as prescribed and reduce re‑hospitalizations by prioritizing high‑risk patient visits. Continuity of Care: Coordinate multidisciplinary care and ensure proper admissions, transfers, and discharges. Family Educator/Advocate: Educate patients and families on disease processes using teach‑back methods to ensure understanding.
