• You will be the "clinical hero" who steps in to provide essential support across multiple nursing centers in our region.
• Whether you are covering a leave of absence, supporting a high-census period, or ensuring a nursing center meets crucial regulatory standards, your expertise will ensure our residents receive the highest level of psychosocial care.

• Navigational Support: Acting as the lead architect for discharge planning by connecting residents with community resources and ensuring a safe, confident transition home.
• Crisis & Transition Management: Providing therapeutic interventions to help residents and families navigate the emotional journey of long-term care.
