The Registered Nurse provides essential nursing support to the primary care health team, ensuring appropriate patient care through assessment, planning, implementation, and evaluation processes for a specific patient population. Responsibilities include consulting with staff, providing evidence-based primary nursing services, conducting initial assessments, obtaining vital signs, reviewing medications, and performing specimen collection like phlebotomy.
Candidates must have completed an approved registered nursing program and possess a valid, unencumbered RN license in Ohio, along with at least one year of experience in primary care, medical-surgical, or acute care settings, and current AHA BLS certification. Preferred qualifications include a BSN and bilingual fluency in English and Spanish, along with knowledge of mental illness and chemical dependency concepts.
Consult and collaborate with staff and providers to correctly execute appropriate plan of care. Provide comprehensive and evidence-based primary nursing services, care, and preventative screenings to patients. Provide patient-centered care within the nursing license scope of practice including: initial assessment and intake, induction, stabilization, and maintenance phases of treatment. Obtain vital signs for each patient prior to the provider visit. Conduct thorough and accurate reviews of patient medications, update medication list, and add prescription information to the medication list. Perform procedures and specimen collection, including phlebotomy as needed. Complete appropriate documentation in the medical record to comply with state, federal, and departmental policies when sharing/documenting patient care data. Coordinate both adult vaccines and vaccines for children (VFC) programs and inventory with the organization’s Vaccine Coordinator (VC). Assist in coordination of care for patients with other service lines within and outside Signature Health. Coordinate excellent patient care with staff of outside healthcare facilities or emergency rooms when necessary. Completes appropriate prior authorizations and patient assistance forms. Ask prompted health related questions and follows structured screening protocol Conduct nasopharyngeal and/or anterior nare specimen collection for the purpose of screening patients for COVID-19 Administer vaccines or any necessary medications. Responsible for triaging patients for COVID screening and assisting with relaying patient test results Provide patient education regarding COVID symptoms, prevention, testing and when to report to the Emergency room. Perform laboratory specimen collection, reconciliation of lab specimens with laboratory orders and batch orders for laboratory pick-up after hours. Follow all requirements related to personal protective equipment, hand hygiene, cleaning of work station and physical distancing practices. Assist with clinic set-up, tear-down and sanitation under the supervision of the Clinic Manager. Exhibit a non-judgmental, empathetic and a fully supportive approach when communicating with patients, families, and staff. Ensure sensitivity to cultural diversity and maintains age-related competence for the population serviced. Monitor operation of medical equipment by verifying equipment availability, completing preventive maintenance requirements, following manufacturer's instructions, troubleshooting malfunctions, calling for repairs, maintaining equipment inventories, and evaluating new equipment and techniques. Maintain your individual clinical and professional knowledge and skill through formal and informal educational experiences. Assist as back up with staffing for direct patient care and Provider assistance. Create a friendly and professional work environment and maintain clear communication with co-workers, referents or designated liaison person. Maintain professional licensure and requirements as designated by the Ohio Board of Nursing. Upholds and ensures compliance with and attention to all agency policies and procedures. Other duties as assigned.
KNOWLEDGE & EXPERIENCE: Completion of an approved program in registered nursing required. Valid unencumbered RN license in the state of Ohio required. 1+years’ experience in a primary care, medical-surgical or acute care setting required. American Heart Association (AHA) Basic Life Support (BLS) certification required. Bachelors of Science in Nursing (BSN) preferred. Bi-lingual in English and Spanish preferred. A working knowledge of mental illness and the disease concept of chemical dependency. Working knowledge of legal regulations and best practices in healthcare. Excellent organizational and problem-solving skills. Proficient with technology and electronic medical records.
Work is normally performed in a typical interior/office/clinical work environment. While hours of operation are generally standard, flexibility to work evenings and extended hours may be required. Requires periods of sitting, standing, telephone, and computer work.
