The Occupational Therapist will deliver skilled in-home therapy services following physician orders and an individualized plan of care, focusing on improving or minimizing residual physical disabilities to return the individual to productive living within their capability. Responsibilities include initial assessment, plan maintenance program development, modifications, reassessments every 30 days, and consulting with physicians regarding treatment changes.
Candidates must be graduates of an accredited Occupational Therapy curriculum or certified by passing the National Certification examination, and must currently be licensed by the Board of Occupational and Physical Therapy Examiners in the practicing state. A minimum of one year in an acute care setting is required, with two years of Home Health experience preferred, along with access to reliable transportation.

We’re looking for a compassionate Occupational Therapist (OT) to provide intermittent in-home therapy that truly makes a difference. In this role, you’ll deliver skilled occupational therapy services in accordance with physician orders and an individualized plan of care, with the support and guidance of our Client Services Director..

Improve or minimize residual physical disabilities of the patient. Return the individual to optimum and productive living within the patient’s capability. Periodically participate with all other home care personnel in patient’s care planning. Provide prescribed occupational therapy and all skilled procedures. Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days. Consult with physicians regarding change in treatment.
