The Physical Therapist will visit patients in their homes to conduct one-on-one sessions, completing initial and ongoing assessments to determine physical, psychosocial, and environmental needs. Responsibilities also include charting observations, administering prescribed medication, and educating patients and families on home health care strategies.
Candidates must be graduates of an accredited nursing school (Diploma, Associate, or Bachelor's) and hold a current state license as an RN, along with proof of current CPR certification. Essential soft skills include a compassionate and friendly demeanor, excellent communication abilities, and possessing a current driver's license with reliable transportation.
