The RN Case Manager will conduct initial Start of Care visits to assess patient needs and develop individualized care plans, followed by regular reassessments to monitor progress. Responsibilities also include educating patients and families, coordinating care with other professionals, maintaining accurate documentation, and supervising home health aides.
Applicants must possess an active Registered Nurse License in Ohio, proof of U.S. work authorization, and reliable transportation. A recent Tuberculosis screening is also required for this role.
