What you will do
Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
Provide hands-on skilled nursing care, including wound care, medication management, injections, and chronic disease management
Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
Collaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care
What you will have
Active Registered Nurse (RN) license in the state of practice
Experience in home health, skilled nursing, or a related healthcare setting preferred
