The Licensed Social Worker is responsible for planning, organizing, and developing programs and services to meet the emotional and social needs of residents, while also assessing and supporting these needs on an individual basis. This role involves maintaining accurate documentation and collaborating with interdisciplinary teams to ensure continuity of care.
Candidates must be a Licensed Social Worker in good standing and have completed secondary education at an accredited school of Social Work. Prior experience in a supervisory capacity is preferred, and experience in geriatric, adult, or long-term care social work is strongly desired.
We are seeking a PT Licensed Social Worker who is able to dedicate around 8 hours per week to our center. The Licensed Social Worker plays a key role in supporting the emotional and social well-being of our residents. Under the direction of the Administrator, this role is responsible for planning, organizing, and developing programs and services that meet the medically related emotional and social needs of each resident. The ideal candidate is professional, compassionate, and committed to person-centered care.
