About the role

Physical Therapist (PT) / Occupational Therapist (OT)

We are seeking dedicated and compassionate Physical Therapists and Occupational Therapists to join our team at Community Home Health Care in Fort Wayne, IN. In this role, you will provide exceptional rehabilitative care to patients in their homes, helping them improve their mobility, independence, and overall quality of life.

Responsibilities

  • Conduct thorough patient evaluations to develop individualized treatment plans.
  • Deliver personalized therapy sessions focusing on improving strength, coordination, and daily living skills.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Requirements

  • Valid driver’s license and reliable transportation.
