The Physical Therapist performs evaluations, develops and implements the plan of care, provides physical therapy services and interventions according to APTA guidelines, and participates in collaborative patient care and discharge planning.
Candidates must be graduates of an accredited Physical Therapy program and hold a current Illinois Physical Therapy license, demonstrating necessary clinical skills and competencies.
Performs Physical Therapy (PT) evaluation, reassessment and consultation; provides physical therapy plan of care, goals, discharge planning, patient and significant other (SO) education; renders physical therapy services, interventions, use of therapeutic modalities within the practice guidelines of the American Physical Therapy Association (APTA) and other regulatory agencies; and participates in collaborative patient care services.
