The MDS Coordinator provides oversight of the assessment and care planning process within the nursing department, ensuring MDS accuracy and timely submission according to CMS RAI guidelines. This role also involves coordinating resident admissions and initial services with the Interdisciplinary team, with potential for general nursing care as directed.
Candidates must be a graduate of an accredited school of nursing and hold a current, valid Registered Nurse (RN) license in Hawaii, along with working knowledge of MDS and RAI. Required qualifications also include basic computer proficiency, valid CPR/First Aid/AED certification, and availability for varied schedules including weekends and holidays.

Provides oversight of the assessment and care planning process in the nursing department. The MDS Coordinator is responsible for ensuring that MDS (Minimum Date Set) is accurate, complete, follows the CMS MDS RAI guidelines (for Medicare and Medicaid) and is submitted to the appropriate agency or provider in a timely manner. Coordination of resident admissions and initial services with members of the Hi’olani Interdisciplinary team (IDT). May provide general nursing care to residents in the Comprehensive Nursing units at Hi’olani Care Center on an as needed basis as directed by the Director or Assistant Director of Nursing Services. THIS POSITION IS 100% ON SITE, NO REMOTE OR HYBRID WORK AVAIALABLE. SCHEDULE TO BE DETERMINED BY MANAGEMENT AND DEPARTMENT NEEDS.
