The Social Worker will develop social histories, create individualized care plans to support resident goals, and provide individual and group support to residents and families during times of crisis or transition. Responsibilities also include participating in care plan conferences, coordinating discharge planning, facilitating staff training, and building relationships with external partners.
Candidates must hold a Bachelor’s Degree in Social Work (BSW) or equivalent certification and possess effective communication and interpersonal skills. Experience working in long-term care settings is required, along with compassion, organization, and commitment to resident well-being.
