About the role

The Licensed Social Worker plans and organizes programs to meet the emotional and social needs of residents in a long-term care setting. They also collaborate with interdisciplinary teams to manage care planning, discharge processes, and maintain accurate medical records.

Social worker

We are seeking a Licensed Social Worker. The Licensed Social Worker plays a key role in supporting the emotional and social well-being of our residents. Under the direction of the Administrator, this role is responsible for planning, organizing, and developing programs and services that meet the medically related emotional and social needs of each resident. The ideal candidate is professional, compassionate, and committed to person-centered care.

Key Responsibilities

  • Assess and support the emotional and social needs of residents on an individual basis.
  • Maintain accurate documentation in resident medical records according to facility policies and federal/state regulations.
  • Collaborate with interdisciplinary teams to coordinate continuity of care.
  • Serve as a liaison between residents, families, and community agencies.
  • Participate in care planning and discharge planning processes.
  • Other duties as assigned by the Administrator.

Qualifications

  • Must be a Licensed Social Worker in good standing.
  • Completion of secondary education at an accredited school of Social Work.
  • Prior experience in a supervisory capacity is preferred.
  • Experience in geriatric, adult, or long-term care social work is strongly desired.
  • Excellent communication and interpersonal skills with the ability to work professionally and cooperatively with residents, families, and team members at all levels.
