The Certified Nursing Assistant will deliver compassionate, high-quality care to patients across the lifespan, partnering with providers to ensure coordinated treatment. Key duties include supporting patients/families, accurately documenting care in the EHR, and participating in quality improvement initiatives within the Patient-Centered Health Home model.
Required qualifications include an active CNA certification in good standing with North Carolina, a High School Diploma, and current CPR/BLS certification along with a valid Driver’s License. Preferred qualifications include one year of clinical experience, especially in primary care or community health settings, and familiarity with underserved populations.
