The Care Coordinator is responsible for coordinating services to help older adults maintain their independence. This includes determining eligibility, creating care plans for the Illinois Department on Aging, conducting home visits and assessments, and educating clients and families on available resources.
Candidates must possess an RN license, a BSN, or a BA/BS degree in social science, social work, or a related field. One year of relevant program experience working with the elderly can substitute for up to four years of college education.

Shawnee Health is seeking a Care Coordinator covering Carterville and nearby towns. This position reports directly to the Care Coordinator Supervisor.
Paid Time Off Call & Incentive Compensation Care Coordinators, Case Manager Assistants, and Adult Protective Services Case Workers and Specialists are eligible for call compensation.
