The Registered Nurse Case Manager will complete initial and ongoing nursing assessments of in-home clients, performing specialized procedures and participating in the development and evaluation of the patient's plan of care. Responsibilities also include overseeing the case load through ongoing interdisciplinary care coordination and performing home visits to evaluate care quality.
Candidates must be a graduate of an accredited nursing program with an active RN license in Colorado and possess a minimum of one year of nursing experience, preferably in home healthcare. Required qualifications also include CPR Certification and the ability to pass a background check and drug test, including testing for THC.
Visit Rates: $50–$130 per visit | $37 per point for the first 4 weeks during training
