The Registered Nurse Case Manager will complete initial and ongoing nursing assessments of in-home clients, performing specialized procedures and participating in the development and evaluation of the patient's plan of care. Responsibilities also include overseeing the case load through ongoing interdisciplinary care coordination and performing home visits to evaluate care quality.
Visit Rates: $50–$130 per visit | $37 per point for the first 4 weeks during training
