Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury, working collaboratively within a home health team to meet the patient's plan of care goals. Responsibilities include observing movements, conducting assessments, documenting progress, modifying care plans, assisting with exercises, and educating patients and families on therapy and self-care.
Candidates must be a graduate of an accredited Physical Therapist Program with an active PT license in Colorado, and CPR Certification is required. Two years of relevant physical therapist experience within the last five years is preferred, along with knowledge of state and federal home health regulations.
