The Occupational Therapist will provide therapy to patients unable to perform daily tasks due to illness or injury, working within a home health team focused on the patient's plan of care and personal goals. Responsibilities include assessing patients, establishing goals, documenting progress, assisting with exercises, suggesting adaptive equipment, assessing fall risks, and educating patients and families.
