The role involves coordinating a patient care delivery system for a designated group of patients to achieve high-quality, cost-effective outcomes across the continuum of care, aiming to optimize the patient's health status. This includes performing duties in an ambulatory setting and assisting complex patients during transitions of care across multiple settings while providing targeted interventions to prevent hospitalizations and emergency room visits.
A minimum of three years of Registered Nurse (RN) experience is required, or equivalent Licensed Practical Nurse (LPN) experience approved by the CNO, including at least one year as an RN, alongside a Bachelor's degree in Nursing (BSN). Candidates must possess effective communication skills, the ability to make independent clinical judgments, experience with basic patient care equipment, and current RN licensure in Nebraska or a compact state, along with Basic Life Support (BLS) certification.
