The Care Manager will conduct thorough client needs assessments, develop tailored support plans, and coordinate with community resources to ensure clients receive necessary assistance. This role also involves serving as the main point of contact, advocating for clients, monitoring plan progress, and maintaining accurate documentation and reports.
Candidates must be fluent in both English and Spanish and possess a valid driver's license and vehicle for extensive field travel. A Bachelor's degree in a related field is preferred, along with proven experience in non-medical care management or social services.
The Care Manager provides Enhanced Care Management services to assigned members. This role focuses on member engagement, care coordination, and documentation in alignment with payer and regulatory requirements.
The Care Manager is responsible for completing required ECM activities accurately, on time, and in accordance with established standards.
