The Care Manager will conduct thorough client needs assessments, develop tailored support plans, and coordinate necessary community resources and services. They will serve as the main point of contact, advocating for clients and monitoring the progress of support plans while maintaining accurate documentation.
Candidates must possess fluency in both English and Spanish, along with a valid driver's license and vehicle for extensive field travel. A Bachelor's degree in a related field is preferred, alongside proven experience in non-medical care management or social services.
The Care Manager provides Enhanced Care Management services to assigned members. This role focuses on member engagement, care coordination, and documentation in alignment with payer and regulatory requirements.
The Care Manager is responsible for completing required ECM activities accurately, on time, and in accordance with established standards.
