The Home Health SOC Registered Nurse will provide home-based care to patients recovering from various conditions, collaborating with a multidisciplinary team to ensure high-quality care delivery. Candidates must be a graduate of an accredited Nursing Program with an active RN license in Illinois and possess a minimum of two years of nursing experience, preferably in home healthcare.
