The Certified Home Health/Hospice Aide provides personal care services under the supervision of an RN or therapist to support patient comfort and independence. This role includes documenting patient observations and promptly reporting any changes to support safe continuity of care.
Candidates must possess an active California Certified Home Health Aide (CHHA) certification, current BLS, and be registered on the Home Care Aide Registry, along with a valid driver's license and insured vehicle. A High School Diploma or equivalent is required, along with experience providing personal care in Home Health and/or Hospice settings.
Shift: Monday–Friday, 8:30 AM – 5:00 PM
