Perform specific treatments and medication administration according to patient care plans. Collaborate with physicians and healthcare teams while maintaining accurate and timely clinical documentation.
Must hold a valid and unencumbered LPN license in the state of service and possess reliable transportation. Required certifications include current CPR and proof of TB results or a chest x-ray.
Performs specific treatments and medication administration as per care plan Functions as a team member demonstrating collaboration with and responsibility to the patient, Physician, and health care team. Identifies issues of concern and demonstrates critical thinking skills. All clinical documentation must be completed accurately and submitted timely per agency policy/guidelines. Reports on time and as scheduled to complete work within the designated time. Adhere to all company policies and procedures outlined in the Employee Handbook Requirements Hold a valid/unencumbered verification/license to practice the job as described in the state where services are provided. Reliable transportation. Provide a copy of current TB results or Chest x-ray. Provide a copy of CPR certification as per agency policy. Be flexible, organized and have the ability to exercise independent and sound judgment to perform the job duties/responsibilities. Physical Requirements Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms, and perform other physical activities as deemed necessary by job duties. Vision and Hearing capabilities within normal limits, with or without aid. Ability to frequently lift and or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable. Ability to perform the anticipated level of physical assistance required to provide safe client care.
