Your Role in Our Mission

Easterseals PORT Health’s Tailored Care Management (TCM) team is committed to delivering whole-person, integrated care that truly makes a difference. As a Tailored Care Manager, you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands-on role with the opportunity to travel throughout Clinton, NC and surrounding counties, helping close gaps in care and improving lives in the community every day.

This full-time role offers a primarily Monday–Friday schedule from 9 AM to 5 PM, with rotating on-call responsibilities. What We’re Looking For

We’re seeking a compassionate, driven professional with a passion for helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after-hours support.

We also require

  • Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. Master’s level degree preferred.
  • Eligible for Qualified Mental Health Professional (QMHP) credentialing
  • Minimum 2 years of experience with mental health and/or substance use populations
  • Proficiency in Microsoft Office and Electronic Health Records (EHR)
  • Valid driver’s license, current auto insurance, and a clean driving record
  • Strong communication skills and the ability to work independently

Requirements

Candidates must possess a Bachelor's degree in a relevant human services field, with a Master's degree preferred, and must be eligible for Qualified Mental Health Professional (QMHP) credentialing. A minimum of two years of experience working with mental health and/or substance use populations is required, along with proficiency in Microsoft Office, EHRs, and a valid driver's license.
