The Case Manager is responsible for coordinating all necessary services to maximize consumer recovery and independence for individuals with mental illness, while also providing clinical and program consultation to direct care staff in residential or clinical settings. Key duties involve ensuring case management compliance, assisting consumers in improving quality of life, and implementing goal-oriented, individualized strategies for obtaining services and supports.
A Bachelor’s degree in a human services field from an accredited institution is required, and preferred qualifications include LLBSW, LBSW, LLMSW, LMSW, or LMFT licensure. Candidates must possess a valid Michigan Driver’s License and prior case management experience in a mental health or rehabilitation setting is preferred.
Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness. In a residential or clinical setting, provide clinical and program consultation to direct care staff. Ensure case management compliance with agency standards and the practice model. Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community. Implement strategies for obtaining services and supports that are goal-oriented and individualized.
