The Community Health Worker acts as a liaison between health/social services and the community to improve service access and quality, building capacity through outreach, education, counseling, and advocacy. Responsibilities include engaging participants for care management programs, providing community outreach like home visits, coordinating services, and documenting client progress for care managers and providers.
Candidates need three years of job-related experience providing medical, mental health, or substance abuse services to individuals with chronic conditions or severe mental illness, or must be a certified Peer Specialist if lacking minimum experience. A high school diploma or equivalent is required, along with maintaining a valid driver's license and transportation for required field travel.
This position is Remote, however; the Candidate will be expected to reside in and travel throughout Grays Harbor, Clallam and Jefferson Counties.

The position is FLSA Non-Exempt and is eligible for overtime and has a 5% annual incentive target based on company, department, and individual performance goals. The base pay actually offered will take into account internal equity and also may vary depending on the candidate’s job-related knowledge, skills, and experience among other factors.
