Public Health Nurses promote and protect the health of citizens through education, prevention services, regulations, and advocacy, fostering partnerships to resolve community health issues. Responsibilities include performing initial and ongoing health status assessments for individuals and families across various settings, providing case management, and ensuring accurate documentation for service delivery and reimbursement.
Candidates must be certified as a public health nurse in Minnesota and hold a current registered nurse license in Minnesota, along with possessing a valid driver's license. The role requires thorough knowledge of nursing practices, public health laws, and theory, alongside strong decision-making and communication abilities.
Promote and protect the health of Cass County citizens and visitors using knowledge from nursing, social, and public health sciences, through education, prevention services, regulations, and advocacy. Foster a partnership with many organizations and community groups to address and resolve health issues and concerns. The most important impact Public Health Nurses have on the County is the nurses' ability to empower individuals, families, communities, and systems to be self-sufficient in meeting their health and wellness needs.
Performs initial and ongoing assessments and evaluations to determine the health status of individuals and families across the lifespan in their home, clinics, and community settings; performs a variety of assessments to determine their risk, exposure, and treatment options linking people to providers and assures the provision of health care when otherwise unavailable. Will provide case management services to clients. Nursing functions may include MnChoice assessment across all ages, obtaining and reviewing medical information to ensure program eligibility. Provide accurate documentation so that client services are efficiently delivered, records follow Agency/State/Federal regulations and reimbursements for services are maximized. Maintain/improve case management/skills, and meet educational requirements set by the Department of Human Services. May require after-hours or weekend coverage in response to public health emergencies. Performs other duties as assigned.
Thorough knowledge of the application of nursing practices. Through knowledge of local, state, and federal public health laws, rules, ordinances, policies, procedures, manuals, regulations, and grant requirements. Considerable knowledge of public health nursing theory. Through skill in using standard office equipment and applicable hardware, and software. Ability to establish and maintain effective relationships with clients, agency staff members, other agency professional representatives, contractors, and the public. Ability to make independent decisions. Ability to communicate effectively, both orally and in writing. Ability to assess client needs and to develop and carry out care plans. Ability to plan, organize, and prioritize workload; ability to respond to emergencies, disasters, and difficult problem-solving solutions. Ability to apply principles of public health to communities, families, and individuals.
Current Certification as a public health nurse in Minnesota and current licensure as a registered nurse in Minnesota. Must possess a valid driver’s license. Public Health Nurse certification in Minnesota must be obtained within six (6) months of employment.
Sedentary to light physical activities requiring stooping, kneeling, crouching, reaching, handling, talking, hearing, and close vision. The employee must occasionally lift and or move up to 35 pounds. Typically, in an office environment.
