include assessing and verifying service coordination needs, planning services, making referrals, monitoring service provision, and advocating for consumers. The role also requires providing after-hours crisis services and maintaining progress documentation.
Candidates must have a Bachelor's degree in any field, or an Associate degree in a relevant human service field with experience working with individuals with IDD, or a High School diploma with two years of relevant experience. The position demands a flexible schedule to accommodate consumer and family needs, including working after-hours, holidays, and weekends.
Bachelor's degree or advanced degree from an accredited college or university (any field) OR Associate degree in a social, behavioral, human service, or health-related field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice with at least one year paid or unpaid experience working with individuals with IDD is preferred OR High School diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
Bilingual in Spanish & English a plus.
$19.00/hourly, 8-5 (Mon-Fri)
