This role involves providing direct care in a therapeutic environment, supporting clients transitioning from psychiatric hospitals by helping them learn skills for community and independent living. Responsibilities include participating in client assessments, coordinating comprehensive care, monitoring medication compliance, and providing transportation and advocacy for medical appointments.
Candidates must possess a high school diploma or GED, with a Bachelor's degree in a related field preferred, and must be willing to obtain required certifications and training. A valid driver's license with minimal points and access to an insured vehicle are mandatory, along with availability for varied shifts including days, nights, weekends, and holidays.
The Segue Residential Transition Program [https://www.sheppardpratt.org/care-finder/crisis-residential-program/segue-residential-transition-program/] assists with helping individuals transition out of state psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills.
Full Time Schedule Tuesday-Saturday 7am to 3pm

This is a direct care opportunity to provide support for mental health treatment. You will provide a safe and therapeutic environment that encourages the overall independence and wellbeing of your clients.
As you spend time with your clients, you will find yourself developing a rapport with your clients where you become part of their trusted support system. You will see their wins as your wins as they work their way through the program and cheer them on as they progress in their recovery!
