The Advanced Practitioner will provide comprehensive primary care services in an outpatient setting, addressing acute concerns, conducting wellness visits, and managing chronic conditions. Responsibilities also include educating patients, coordinating care with specialists, utilizing telemedicine, and maintaining thorough documentation.
Candidates must be a Certified Nurse Practitioner (NP) or Physician Assistant (PA) with at least two years of primary care experience, preferably in internal medicine, family medicine, or geriatrics. Fluency in Mandarin or Cantonese is required, along with experience with Medicare patients and strong knowledge of medical documentation, coding (ICD-10, HCC), and risk adjustment.
