The Patient Navigator - RN guides and supports patients and families throughout their care continuum by coordinating services, overcoming barriers, and facilitating communication between all involved parties. This role involves assessing patient needs, providing education, acting as an advocate, and coordinating referrals to necessary support services.
Candidates must possess 3-5 years of clinical nursing experience and hold an active Registered Nurse (RN) license with Basic Life Support (BCLS) certification. Preferred experience includes 1-3 years in patient navigation, care coordination, or case management.

The Patient Navigator collaborates with interdisciplinary teams, community resources, and support services to ensure a seamless and patient-centered healthcare experience.
3-5 years of clinical nursing experience required 1-3 years of experience in patient navigation, care coordination, or case management preferred
In-depth knowledge of patient navigation, care coordination, and nursing principles. Strong communication and interpersonal skills to interact effectively with patients, families, and healthcare providers. Ability to assess and address barriers to care, demonstrating empathy and problem-solving skills. Knowledge of community resources and support services to address patient needs holistically. Proficiency in patient education, including developing materials and conducting teaching sessions. Organizational skills to manage multiple priorities and coordinate services across the continuum of care. Familiarity with insurance processes, financial assistance programs, and healthcare regulations.
