The role involves performing patient assessments and collaborating with the care team to create and implement individualized plans of care, including making necessary referrals to other hospice disciplines. Responsibilities also include maintaining accurate patient records, teaching care techniques to families, communicating with the clinical manager, and performing assigned on-call duties.
Candidates must possess at least one year of nursing experience along with a current Registered Nurse (RN) license specific to the assigned work state. A current CPR certification, a valid driver's license, reliable transportation, and liability insurance are also mandatory qualifications for this position.
