The Medical Social Worker will establish care plans aligned with the physician's treatment, provide necessary crisis intervention, and assist caregivers in understanding factors affecting the patient's medical improvement. Responsibilities also include educating patients and caregivers, making community referrals for resources, and resolving caregiver-patient relationship issues.
Candidates must possess a Master’s degree from a CSWE-approved school and be licensed to practice in the serviced state, including Indiana certification under IC25-23.6-4-3. A minimum of one year of social work experience in a healthcare setting and current CPR/BLS certification are also mandatory requirements.
