The Physical Therapist will plan, implement, and evaluate care plans to restore or maintain patient well-being across all age groups, organizing and conducting treatment based on medical referrals and evaluations. Essential functions include performing clinical assessments, documenting the Plan of Care, applying infection control concepts, and providing appropriate treatment for various disorders.
Candidates must possess a Bachelor of Science in Physical Therapy from an accredited program and have two years of experience as a PT in a clinical setting, with home health experience being preferred. Required credentials include a current state professional PT license, a valid CPR certificate, a valid driver's license, reliable transportation, and auto liability insurance.

Promote /exemplify Company mission, vision and values at all times. Perform clinical assessments; identifies patient needs and appropriateness of client. Document, prepare/revise and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis. Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA). Plan and provide appropriate treatment for neuro-muscular neurological and orthopedic disorders based on medical referral and therapy assessment. Assist/instruct client/family in transfer techniques appropriate to client abilities. Instruct on use of equipment. Provides effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case Conference/Team Meetings. Monitors assigned cases to ensure compliance with the requirements of third-party payers. Completes appropriate documentation in a timely manner to assure compliance with company policy. Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency. Meet mandatory continuing education requirements of the Agency/licensing board. This description is a general statement of required essential functions performed on a regular and continuous basis. Experience: Two (2) years experience as a PT in a clinical care setting. Preferred, Home health experience.
Therapy skills as defined as generally accepted standards of practice. Excellent interpersonal skills and ability to communicate effectively. Demonstrates proven decision-making skills. If required to make visits, Hepatitis profile. Must read, write and comprehend English.
