The role is responsible for assisting patients in maintaining, restoring, and improving their movement, activity, and overall function. This includes providing necessary interventions, conducting regular reexaminations, modifying treatment plans as needed, and performing discharge planning.
Candidates must be graduates of an accredited physical therapy educational program and hold a current physical therapy license for the assigned state. A minimum of one year of related physical therapy experience is required for those entering home health care, though less experience may be acceptable with regional approval and mentoring.
Responsible for helping patients maintain, restore and improve movement, activity and function. Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning. Other duties as assigned.
