Duties & Responsibilities
Assist a Nurse Case Manager in providing direct patient care.
Initiate communication with team members, HHAs, and other healthcare providers to facilitate optimal care.
Instruct primary caregivers, patients, volunteers, and employed caregivers in care provision based on the Plan of Care as requested by the Nurse Case Manager or Team Leader.
Perform nursing procedures within the scope of a Licensed Vocational Nurse.
Maintain current documentation in a computerized clinical documentation system according to organization policies, procedures, and applicable laws and regulations.
Required Qualifications
Graduate of an approved and certified LVN program.
Current and unencumbered license to practice as an LVN in the state.
Ability to make quick judgments and work independently.
Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines.
At least one year of LVN clinical experience; home health experience preferred.
